Request to join an active research study as a site from an invite link

The link must include a research study and a site ID:
researchStudy— the study you are requesting to join (researchStudyIdis also accepted)siteId— the site ID for this request (siteIDis also accepted)
Without both values, the page sends you to Labsuite Hub home.
Purpose
This page is how an invited organization asks to join an active research study as a site. You confirm the study, enter the site’s contact details, and submit a request for sponsor review.
What you can do
- See the research study you are requesting to join
- Enter the site name and address, prefilled with your organization name
- Add a phone number and an optional fax number
- Submit the enrollment request
- See the site ID from the invite link
Permissions
Page access — Requires a signed-in user with an active organization. Anyone who is not signed in is sent to sign-in and returned to this invite afterward. Anyone without an active organization is sent to organization onboarding and returned here afterward.
No catalog permission applies on this page. Any signed-in member of the active organization can submit the request.
The study must be active. A missing study, a study that is not active, or a link that cannot be read sends you to Labsuite Hub home before the form is shown.
On this page
A centered card reads You are requesting to join the Research Study, then the study name. When the study has no name, the heading is Research study.
Site details
Site Name starts as your organization name. You can change it. Placeholder Enter site name.
Street Address, City, State, and ZIP Code are required. State accepts two characters (placeholder NY). Placeholders for the others are Enter street address, City, and 12345.
Phone Number is required (placeholder (555) 123-4567). Fax Number is optional (placeholder (555) 123-4568).
Fields stay locked while the request is sending.
Submit
Submit Request sends the enrollment. While it is sending, the button reads Submitting….
On success, the page confirms Enrollment request submitted. It is pending sponsor review. and opens Labsuite Hub home. The request stays pending until the sponsor reviews it.
The card footer shows Your Site ID is followed by the site ID from the invite link.
A request is declined when:
- The research study cannot be found
- The study is not active (Sites can only request enrollment in active studies)
- That site ID is already used on this study (A site with this Site ID already exists for this study)
- This organization already has a site on this study (This organization already has a site for this study)
An organization can have only one site on a given study. If submission fails for another reason, the page shows Failed to submit site request.
